Your insurance policy is a contract between you and your insurance company. We are not a party to that contract. Your payment is your responsibility, whether your insurance pays or not. In the event we do accept assignment of benefits, we will bill your insurance company, and you will be responsible for the balance on or before your next visit. If your insurance company has not paid your account in full within 45 days, payment is expected no later than 15 days after you receive your bill. If your bill has been forward to the collection agency, all medical services will be terminated.
We accept assignment of insurance benefits for Medicare. If your second insurance pays the 20 percent to the provider, we will bill it. If you do not have secondary coverage or your secondary insurance does not pay, we do require 20 percent of the bill to be paid at time of service. Please be aware that some and perhaps all of the services provided may be non-covered services and not considered reasonable and necessary under the Medicare program and/or other medical insurance.
Regarding insurance plans where we are a participation provider: All co-pays are due prior to treatment. There will be a $2 fee for all late co-pays. Deductibles and any balances are due at time of services. In the event that your insurance coverage changes to a plan where we are not participation providers, we do require payment in full at time of service.
There will be a $40 fee for all letters to be written by the doctor and $25 for forms to be completed. Please allow seven days for completion.
